Graham Thorpe's care had 'failings' in last months of his life, says coroner

Thorpe had previously attempted suicide and had communicated to his wife his desire for help in ending his life before he died.
Healthcare professionals did not perceive Thorpe to be in crisis despite his statements and missed appointments, indicating a failure in monitoring his condition.
